| (a)      Applications for intoxicating liquor licenses shall be made to and filed with the
<br /> 			City Clerk. On sale applications shall be made on forms prescribed by the City Council.
<br /> 			Off sale applications shall be made on forms prescribed by the Liquor Control
<br /> 			Commissioner. All forms shall be supplied by the City Clerk. The Council may require
<br /> 			applicants to furnish information in addition to that contained in the written forms. If
<br /> 			required under the provisions of Minnesota Statutes Ch. 340A, a liquor liability insurance
<br /> 			policy shall accompany each application for a license.

<br /> 			(s       Every application for a license to sell intoxicating liquor shall state the name of
<br /> 			the applicant, his or her age, representations as to his or her character, with such
<br /> 			references as the City Council may require, the type of license applied for, the business in
<br /> 			connection with which the proposed license will operate, its location, whether the
<br /> 			applicant is owner and operator of the business,how long she or he has been in that

<br /> 			such information, the application shall be in the form prescribed by the Liquor Control

<br />											MEMENTMeCoity
<br /> 			Council and/or its Committee shall have at least thirty(30) days from and after receipt of
<br /> 			the application and surety bond for review and examination of the application and surety
<br /> 			bond prior to granting or denying issuance of a license. No license shall be valid until
<br /> 			approved, where necessary by the State of Minnesota Liquor Control Commissioner.
