PERFORMANCE APPRAISAL POLICY <br /> CITY OF PEQUOT LAKES <br /> Purpose: The City of Pequot Lakes has determined that a <br /> Performance Assessment Process is needed to insure that <br /> employees have a good perspective on their personal performance <br /> and where improvement are needed, there is a process to provide <br /> the needed improvements. <br /> Policy: All employees of the City of Pequot Lakes will have their <br /> performance reviewed in the prescribed manner at least once every <br /> calendar year. The manager/supervisor/Council liaison will <br /> provide the employee with a minimum of two week notice of the <br /> review session and will give the employee written documents for <br /> review, including a personal copy of the report. The review will be <br /> based on the employee's job description as well as any goals that <br /> were established for the preceding time period. <br /> Operating Procedure: Forms will be created that will provide <br /> structure to the review process. Managers and Supervisors will <br /> have the option to modify these documents to best fit the nature of <br /> the individual's position and duties. <br /> Copies of all reviews will, after signature by both the <br /> manager/supervisor/council liaison and employee, be retained in <br /> the employee's personnel file. A copy of the review forms will be <br /> given to the employee. The employee will be entitled to prepare a <br /> statement regarding the review, which will be attached to the <br /> review documents. <br /> Unsatisfactory Performance Appraisal: In the event that a <br /> performance appraisal is unsatisfactory, as determined by the <br /> �, supervisor and their Council liaison, the employee will be kept at <br />
