i <br /> PERFORMANCE ASSESSMENT PROCESS <br /> CITY OF PEQUOT LAKES <br /> (Draft of Poiicy) <br /> Purpose: The City of Pequot Lakes has determined that a <br /> Performance Assessment Process is needed to insure that <br /> employees have a good perspective on their personal performance <br /> and where improvements are needed; there is a process to provide <br /> the needed improvements. <br /> Policy: All employees of the City of Pequot Lakes will have their <br /> performance reviewed in the prescribed manner at least once every <br /> calendar year. The manager/supervisor will provide the employee <br /> with a minimum of two week notice of the review session and will <br /> give the employee written documents for review,_including a <br /> personal copy of the report. This review will be based on the <br /> employee's job description as well as any goals that were <br /> established for the preceding time period. <br /> Operating Procedure: <br /> Forms will be created that will provide structure to the review <br /> process. Managers and Supervisors will have the option to modify <br /> these documents to best fit the nature of the individual's position <br /> and duties. <br /> Copies of all reviews will, after signature by both the <br /> Manager/Supervisor and employee, be retained in the employees <br /> personnel file. A copy of the review forms will be given to the <br /> employee. The employee will be entitled to prepare a statement <br /> regarding the review, which will be attached to the review <br /> documents. <br />
