Should there.be any change in benefit coverage under the policy which are imposed by the <br />insurance carrier without an option to the City and which are applicable to all City employees, the <br />City will notify the Union as soon as practicable of the changes in group insurance policy and <br />will, at the request of the Union meet and negotiate any changes. <br />14.2 Dental, Vision Insurance. If the bargaining unit, as a group, chooses to have group dental and <br />vision insurance coverage for employees, employees may participate upon payment of the <br />monthly premium payment by all bargaining unit employees. <br />14.3 Employees who work an average of thirty (30) hours per week or more shall receive employer <br />contributions toward medical premiums. <br />14.4 Retirees. The Employer will pay on behalf of an employee who retires from the City at a time <br />when the employee is eligible for and will immediately begin receiving a PERA retirement <br />annuity, a portion of the premium for continued coverage under the insurance program covering <br />this bargaining unit, for up to three (3) consecutive years beginning with the employee's <br />retirement from the City, or until the employee is eligible for Medicare, whichever occurs sooner, <br />as follows: <br />a. With 25 or more years of service to the City of Pequot Lakes - premium contribution in <br />an amount equal to the monthly premium contribution made by the City on behalf of <br />active employees under this bargaining unit's policy. <br />b. With 15 to 25 years of service to the City of Pequot Lakes - fifty percent (50%) of the <br />amount of monthly premium contribution made by the City on behalf of active <br />employees under this bargaining unit's policy. <br />Section 14.4 shall not apply to employees hired after January 1, 2013. <br />14.5 Claims Against Employer. It is understood that the Employer's only obligation is to provide and <br />make available to employees an insurance policy and to pay such amounts as set forth herein, and <br />no claims shall be made against the Employer as a result of a denial of insurance benefits by an <br />insurance carrier. <br />14.6 Life Insurance. The Employer shall provide and pay the premium for a $15,000 life insurance <br />policy for each employee. <br />14.7 Worker's Compensation. If an employee is disabled from working due to an injury sustained for <br />which the employee is receiving workers' compensation benefits under the Employer's workers' <br />compensation insurance plan, the employee may continue to be covered under the Employer's <br />group health insurance under the same conditions and at the same level of Employer contribution <br />as would be provided if the employee was in active service, for a period of up to six (6),months. <br />14 <br />
