TDS <br />Addendum — ManagedrP Statement of Wort{ and <br />TDS Internet Access Service Level Agreement (SLA,) <br />up to (2) hours of training per location at no additional charge. Additional training can be available at an hourly rate as <br />requested by (lie Customer.. <br />• Concurrent Voice Calls — The service allows for a certain number of concurrent voice calls depending on the bandwidth <br />provided with the service. The number of current calls and bandwidth is as follows: <br />• The 15M service allows for up to 14 concurrent voice calls <br />• The 3.OM service allows for up to 30 concurrent voice calls <br />• The 6.OM service allows for tip to 60 concurrent voice Galls <br />• The 12.0M service allows for up to 120 concurrent voice calls <br />• The 25.OM service allows for up to 250 concurrent voice calls <br />• The 10.0M service allows for up to 100 concurrent voice calls (Monticello,MN Only) <br />• The 100.OM service allows for up to 1000 concurrent voice calls (Monticello,MN Only) <br />Customer Responsibilities <br />• Contact other Service Provider(s) (if other than TDS) to determine whether you have any contractual liabilities. TDS <br />assumes no liabilities associated With your current provider(s). <br />• Provide a single contact to participate in meetings, program gathering and trouble - resolution. <br />• Provide a single point of contact to assist with data information gathering. <br />Provide a single point of contact on the day of implementation and have the data integrator available. If a Customer <br />contact or data integrator responsible for the Customer's LAN is not available on the day of implementation, TDS will <br />have the right to delay the Customer implementation and assess any waived installation fees. <br />• Provide a single paint of contact that will serve as Companv Administrator. The role of the Administrator includes: <br />a) Attending the Administrator- Training and providing all €rural documentation on Auto Attendant recordings, <br />telephone number assignments and routing of Hunt Groups. <br />b) Leading and coordinating the group employee End User Training process <br />c) Resetting Web Portal and Voice Portal passwords for all users (when required) <br />d) Populating and maintaining basic Profile information for users (e.g., first /last names) <br />e) initial and ongoing configuration of all group services including Auto Attendants, Hunt Group services, <br />Account /Authorization Codes, and any other customer- configurable services subscribed to. <br />f) Assistance to users with user - configurable changes. <br />Administrator_ and End User Training - It is your company Administrator's role to take the lead in setting up the systenm <br />to your business needs and coordinating the end user implementation. Their role also includes: <br />a) Attend Administrative training 2 -3 days prior to implementation of managediP services. <br />b) Distribute the "Pre - Installation instructions" to all end users and assist in getting each user ready for the <br />implementation. <br />c) Distribute the "Day of Implementation lnstructions" and "Polycom Phone Quick Reference Guide" to each <br />user's desk on the day of installation. <br />d) Schedule the Group End User Meeting with all employees, at which time the TDS representative Will be <br />available for questions. <br />e) The Administrator is the main contact for all employees during and after the transition to managediP. <br />• Customer assumes responsibility for the installation of the rnanayectlP - Assistant toolbar client on any workstations <br />requiring the application. <br />• Provide unrestricted access to all areas where equipment is to be installed. <br />• Identify equipment locations, cabling summary, floor plans and end user information. <br />• Provide equipment room that meets manufacturer power, grounding, space and environmental specifications. <br />— Unatltitorized disclosure prohibited.�v -1 <br />Version: 041712 <br />Page 2 of 6 <br />
