1. Employees with at least 25 years of service with the City of Pequot <br /> Lakes who have reached the age of 62 and choose to retire are eligible to <br /> participate in the Group Medical Insurance with one hundred percent <br /> (100%) of the premium paid by the City in the amount and terms being paid <br /> to active City employees until the retired employees are entitled to Medicare <br /> benefits. <br /> 2. Employees with at least 15 years of service with the City of Pequot <br /> Lakes and have reached the age of 62 and choose to retire are eligible to <br /> participate in the Group Medical Insurance with fifty percent (50%) of the <br /> premium paid by the City in the amount and terms being paid to active City <br /> employees until the retired employees are entitled to Medicare benefits. <br /> Eligibility for this benefit exists under the following conditions: <br /> A. This benefit is for the employee only and does not cover <br /> dependents. <br /> B. The City will provide this benefit only as long as the City is <br /> providing Group Medical Insurance benefits to all employees. <br /> C. That benefit ends when a qualified beneficiary becomes <br /> covered under another group health plan. <br /> Resignation <br /> An employee wishing to leave the municipal service in good standing except <br /> for mandatory retirement, shall file with his/her department head, or in the <br /> case of no department head, said employee shall file with the City Council <br /> liaison at least fourteen (14) days before leaving, a written resignation <br /> stating the effective date of the resignation and the reason for leaving. <br /> Failure to comply with this procedure ma be considered cause for denying <br /> the employee future employment by the ' <br /> _ <br /> Unauthorized absence from work for a period of three (3) consecutive <br /> working days may be considered by the Department Head or City Council as <br /> a resignation without benefits. <br /> 23 <br />
