HEALTH INSURANCE <br /> Regular Full-Time Employees <br /> Full-time employees shall be eligible for health insurance provided by the <br /> City. The City may, at its option, require employees to participate in the <br /> payment of the premium. Annually, upon notification by the insurer of <br /> premium changes, the City may determine the amount to be paid by the <br /> employee. In the event that the employee elects not to take this coverage, the <br /> City shall pay a monthly lump sum of$375 per month. <br /> Regular Part-Time Employees <br /> Regular part-time employees shall be eligible for health insurance provided <br /> by the City on an annualized pro-rated basis provided the employee pays <br /> their share as of the date the premium is paid by the City. For example: if an <br /> employee works 1040 hours annually (50%) the City would pay one half of <br /> the premium that would be paid by a full time employee. In the event that <br /> the employee does not elect to take coverage, the City shall pay a pro-rated <br /> lump sum of the $375 as stated in the full-time employees. <br /> Retirees <br /> This benefit is for employees who retire from the City of Pequot Lakes while <br /> in active service to the City between the ages of 62 and 65 with the <br /> following requirements: <br /> 22 <br />
