Job Description and any goals set from the prior years review. The results of <br /> the review will be noted in a prescribed format and the employee will be <br /> given a copy of this report. <br /> Operating Procedure <br /> Forms will be created that will provide structure to the review process. <br /> Managers/Supervisors will have the option to modify these documents to <br /> best fit the nature of the individual's position. Copies of all review <br /> documents will, after signature by both the Manager/Supervisor and the <br /> employee, be retained in the employees personnel file. Employees will be <br /> given a copy of this report. Employees will be asked to sign the review <br /> document to acknowledge that they have seen the materials. This is not an <br /> agreement to the report. Employees will be entitled to prepare a statement <br /> regarding the review, which will be attached to the review documents. <br /> Supervisor/Managers are encouraged to review the current Job Description <br /> with the employee and where there are changes in the duties, these should be <br /> noted. When there are significant changes, the revised Job Description <br /> should be submitted to the Job Evaluation Committee for possible Job Point <br /> revisions. <br /> Under the Open Meeting law, the City Council may close a meeting to <br /> evaluate the performance of an individual subject to its authority. It must, <br /> however, identify the individual to be evaluated prior to closing a meeting. <br /> At its next open meeting, the reviewing body must summarize its <br /> conclusions regarding the evaluation. (Minn. Stat. 471.705, Subd.lb, c <br /> and d) <br /> 5. PERSONNEL FILES <br /> Content <br /> Each employee shall have a permanent personnel file retained in the office <br /> of the City Clerk. Copies of all pertinent correspondence, annual <br /> performance evaluations, responses to evaluations, initial employment <br /> forms, and other required forms shall be stored in this file. Personnel files <br /> shall be secured and available to the employee, and to members of the City <br /> Council only upon request to the City Council liaison. Requests for <br /> 8 <br />
