December 23, 2009 <br />Pequot Lakes <br />West Lake Street Proposal <br />Page 2 of 4 <br />Tasks I Amount <br />I. Existing Conditions Survey and Feasibility Study <br />After appropriate office research, a field survey will be completed to update <br />topographical information as well as obtain utility locations. As part of this <br />survey, property documents will be researched to determine the extent of <br />City right -of -way and easements in the project area. <br />We will assist the City in acquiring property and/or easements that may be <br />necessary for the reconstruction of West Lake Street. <br />Discussions will be held with MnDOT to determine whether pedestrian <br />crossing facilities and a storm sewer crossing will be allowed as part of the <br />project. <br />If necessary, we will identify property and /or easements for acquisition by <br />the City for use as storm water runoff retention and infiltration areas and <br />assist the City and its attorney in acquiring these. <br />II. Final Design, Construction Documents, & Bidding <br />Upon completion of the Phase I tasks listed above, and after authorization <br />by the City to proceed, we will prepare final plans and specifications. <br />We will prepare a Minnesota Pollution Control Agency (MPCA) National <br />Pollutant Discharge Elimination System (NPDES) General Stormwater <br />Permit as well as the Stormwater Pollution Prevention Plan (SWPPP). The <br />City is to pay any permit fees. <br />We will finalize plans and specifications based on City reviews. <br />We will prepare an Advertisement for Bids and submit to the Construction <br />Bulletin, the Pequot Lakes Echo, and the Brainerd Daily Dispatch. City is <br />to pay any advertisement costs. <br />We will provide copies of Contract Documents to the City to distribute to <br />bidders. We will assist in addressing bidders' questions, issue addenda as <br />required, attend the bid opening, and prepare a tabulation of bids and a <br />recommendation of award. <br />YA2009 Proposals \ — Letter Proposals\Pequot Lakes - W Lake - 20091215.doc <br />$15,800 <br />$14,200 <br />
