INTRODUCTION
Format of this Manual
This manual can be separated into four basic sections:
Section 1. LOCKOUT/TAGOUT PROGRAM
The first section of the manual is the City of(Name) written Lockout/Tagout Program.
Section 2. AN OVERVIEW OF THE REQUIREMENTS
This section explains those things the city must do in order to have a program that is in
compliance with the law. This overview is meant to be a quick reference for
supervisors/managers.
Section 3. FORMS NEEDED FOR COMPLIANCE
In this section are forms (identified as LO/TO for lockout/tagout)that will make the program
easier to manage and maintain. The primary focus of this section is to assist personnel with
required procedures, documentation, and recordkeeping. A checklist of the major issues
involved with a lockout/tagout program is itemized in Appendix V.
Section 4. APPENDICES
The fourth section is a collection of specific information that is designed to assist managers and
supervisors with regulatory compliance issues. The reference materials provided in this section
should be reviewed by an appointed city representative to make sure the specific protocol in this
manual is correct for the city.
OSHA/Safety Assistance 05/2005
LockOut/TagOut
