I SPECIAL PROVISIONS MISCELLANEOUS WORK ON HIGHWAY RIGHT OF WAY(R/W) PERMIT#3A-US-07-16607(Rob Van Valkenburg/Crafter's Mall) Minor tree removal and brushing on R/W for improved business and sign visibility ALL WORK TO BE DONE AS PER TINA MARKESON'S FEBRUARY 26, 2007 MEMO. Approval granted for removal of underbrush, 26 red pine and white pine trees (less than 4"diameter), standing dead trees, and windfalls,with no damages payment required for these removals. Approval also granted for removal of two areas of cedar trees as per the 2-26-07 memo, with$1,800.00 damages to be paid to the State by the applicant for these removals. Site is on TH 371 south end of Pequot Lakes,RP 49.84—50.04, east side. Conditions: 1. All brushing and limbing work must be done by hand, no heavy equipment allowed that can damage adjacent healthy trees and brush that are to remain. Work must be done from off the highway. Do not park vehicles or equipment on highway shoulder or ditch. If any traffic control is required it must be done in accordance with the January 2007 Field Manual. 2. The removed trees must be cut off flush with ground. Stumps can be ground down flush with surrounding ground, do not attempt to pull out stumps or roots. 3. Underbrush may be removed at ground level. 4. Trees that are to remain can be limbed up to a height of 10 feet above ground if so desired. Refer to the enclosed How To Prune Trees brochure. 5. All debris resulting from removal, limbing, and brushing must be removed from and disposed of off the R/W. Pine debris should be properly disposed of within two weeks to prevent pine bark beetle infestation into the area. Follow the guidelines in the enclosed Pine Bark Beetles brochure. 6. Minor topsoil placement for leveling purposes and seeding is allowed. However,no major filling in or regrading of the R/W area allowed,especially near the two red pines. The area can be kept mowed if applicant desires. 7. Photo records of the existing R/W conditions and trees are on file with Mn/DOT. Any excess removals or damage to R/W beyond the scope of this permit will result in this permit being revoked. If so ordered by Mn/DOT restoration options could include replacement trees, landscaping, and fines,with all work done at applicants' expense. Mn/DOT has 75 feet of R/W from the centerline of TH 371 at this location. No business signs, advertising, items for sale,buildings,wells, septics, or other encroachments can be placed, stored or displayed on the R/W. Notify Mn/DOT when work is completed.
