February 23, 2016 WIDSETH <br /> Proposal for WWTF Construction Phase <br /> City of Pequot Lakes SMITH <br /> Page 2 , NOLTING <br /> 11.We will work with the Contractor on draft payment applications, prepare final monthly <br /> payment applications for the City Council to consider, and transmit all necessary related <br /> paperwork between the City and the Contractor. <br /> 12. Towards the completion of the project, we will conduct a walk-through with the City and the <br /> Contractor and prepare a "punch list" of items the Contractor still needs to complete. When <br /> the Contractor has indicated they have completed those items, we will repeat the process. <br /> 13. We will observe the Contractor's start-up of the system, and assist the City as necessary in <br /> starting up the system. <br /> 14. We will collect the Operation and Maintenance (O&M) manuals provided by the Contractor <br /> (and his material and equipment suppliers) and prepare a comprehensive O&M Manual for <br /> the City. <br /> 15.We will address with the Contractor the various paperwork items to close out the <br /> construction Contract. <br /> 16. We will prepare and submit the MPCA Notice of Completion and Certificate of Completion. <br /> 17. We will field survey and prepare"As-Built" drawings and provide those to the City. <br /> Schedule <br /> We have not received a schedule from the apparent low bid Contractor, so the start date of our <br /> services along with the length of our services—mostly in regard to our field observation time—is <br /> an estimate at this time. <br /> Following is a tentative schedule for 2016: <br /> March: Review and execution of construction Contract <br /> April: Pre-construction meeting <br /> April: Review of Contractor's shop drawings <br /> May, June, and July: Construction in the field <br /> July: Startup <br /> August 1, 2016: Completion <br /> August and September: Closeout, "As-Built"drawings, etc. <br /> Fee <br /> Our fee is based on the above anticipated schedule. <br /> Note that the level and costs of our services is very dependent on the performance of the <br /> Contractor, and to some degree, on weather and other factors outside of our control. <br /> We propose to perform the above tasks on an hourly basis, in accordance with the attached fee <br /> schedule, for an estimated amount of$84,500. <br /> If we need to provide services other than those listed above, or if we need to spend more time <br /> with the Contractor than we originally anticipated, we will review such situations with the City <br /> staff and then with the City Council. <br /> NAMarketing Dept\Proposals\00 Letter Proposals\2016 Letter Proposals\Pequot Lakes-WWTF Constr Phase.doex <br />
