June 15,2015 WIDSETH <br /> Proposal for Pequot Blvd—W. Lake to Ramsey Sts. <br /> City of Pequot Lakes SMITH <br /> Page 3 , NOLTING <br /> 8. Design a new storm sewer system of catch basins, piping, and storm sewer manholes to <br /> provide for positive drainage at the `south to southeast' bend. <br /> 9. Design the road removal and replacement. <br /> 10. It appears we will be very close to possibly disturbing an acre, thus, we envision the City <br /> and construction Contractor needing to obtain a NPDES General Stormwater Permit from <br /> the Minnesota Pollution Control Agency (MPCA). We will prepare a Stormwater Pollution <br /> Prevention Plan (SWPPP). <br /> 11. Prepare construction drawings, including typical sections, plans, profiles, details, erosion <br /> control sheets, work zone traffic control sheets, and cross sections as needed. <br /> 12. Prepare construction specifications/Contract Manual. <br /> 13. Estimate quantities and prepare a construction cost estimate. <br /> It is important to note that as with any construction, there may need to be modifications <br /> made to the quantities in the field. As such, we recommend the City budget a <br /> contingency of about 10% of the contractor's bid. <br /> 14. Complete a Gopher State One Call, as required by State Law, prior to the bid. We will also <br /> distribute plans to local utility company contacts. <br /> 15. Prepare an Advertisement for Bids, and distribute plans on QuestCDN. <br /> 16. Attend the Bid Opening and review bids with the City. <br /> We have always appreciated the capabilities of your staff to observe and administer a <br /> construction contract, and will only assist as needed. <br /> We will wait to provide you with a separate proposal for construction phase services (staking, <br /> observation, and administration) once a Contractor is selected and City staff know their <br /> availability to handle the construction phase. <br /> Schedule <br /> The schedule is to design this year, bid this coming winter, and—if the bids are acceptable - <br /> construct in 2016. <br /> Fee <br /> We propose to perform the above tasks on an hourly basis, in accordance with the attached fee <br /> schedule, for an estimated amount of$28,100. <br /> Services that need to be performed in addition to the tasks listed above will first be reviewed for <br /> scope and effort with the City staff, Council liaison, or Council and charged in accordance with <br /> our 2015 fee schedule. <br /> We plan on being at your July 7th City Council meeting to discuss this proposal and, if needed, <br /> N:\Marketing Dept\Proposals\.Letter Proposals\2015 Letter Proposals\Pequot Lakes-Pequot Blvd-20150615.docx <br />
