13  <br />  <br />5) Safety Vest (bulletproof) <br />6) Duty weapon/holster <br />7) Initial sets (2) of uniforms for new employees to include shirts (summer, winter), pants, <br />jacket/coat, training uniform shirt and pant. <br /> <br />All city-issued equipment and property shall be returned upon severance from service. All <br />equipment purchased with the uniform allowance which is of the same type as the equipment <br />listed in-this Section 12.3 shall also be returned upon severance from service. <br />12.4 In the event there are changes to a specific style or color of uniforms and/or equipment, and it is <br />mandated by the City of Pequot Lakes, the City shall purchase all necessary uniforms (minimum <br />of two sets) and/or equipment. Should a uniform become damaged while in the duty to the City it <br />shall be replaced at no cost to the employee or a decrease in the employee's annual uniform <br />allowance. <br /> <br />ARTICLE 13 <br />INSURANCE <br /> <br />13.1 Health and Hospitalization Insurance, The Employer shall provide a group major medical plan for <br />its employees and their dependents. The employer shall contribute eighty-five (85%) of monthly <br />insurance premium and the employee shall pay fifteen (15%) premium. The benefit level of <br />coverage shall not be reduced from Health Partners$3200-100% HSA Plus Embedded Gold SE <br />Open Access <br />Effective January l, 2024 the City's monthly contribution into the Employees 6 “HSA" accounts <br />shall be: <br />Single $200 <br />Family $325 <br />Should there.be any change in benefit coverage under the policy which are imposed by the <br />insurance carrier without an option to the City and which are applicable to all City employees, the <br />City will notify the Union as soon as practicable of the changes in group insurance policy and <br />will, at the request of the Union meet and negotiate any changes. <br />13.2 Dental, Vision Insurance. If the bargaining unit, as a group, chooses to have group dental and <br />vision insurance coverage for employees, employees may participate upon payment of the <br />monthly premium payment by all bargaining unit employees. <br />13.3 Employees who work an average of thirty (30) hours per week or more shall receive employer <br />contributions toward medical premiums. <br />13.4 Retirees. The Employer will pay on behalf of an employee who retires from the City at a time <br />when the employee is eligible for and will immediately begin receiving a PERA retirement
