shift would reflect use of eight (8) hours. Sick leave use of eight (8) hours or less would remain <br />hour for hour. The eighty percent (.8) hour conversion only applies to full shifts. <br />10.2 Upon separation from employment, due to retirement, voluntary or involuntary termination for <br />whatever reason, all full-time and regular part-time employees with five to nine (5-9) years of <br />service to the City of Pequot Lakes shall receive monetary compensation in the amount of ten <br />percent (10%) in cash of their unused sick leave, up to 960 hours. Employees with ten to fifteen <br />(10- 15) years of service shall receive monetary compensation in the amount of twenty-five <br />percent (25%) in cash of their unused sick leave up to 960 hours. Employees with sixteen (16) or <br />more years of service shall receive monetary compensation in the amount of tweWyfive percent <br />(35-0,;)- fifty percent (50%)_ in cash of their unused sick leave up to 960 hours. <br />10.3 An employee shall be granted sick leave with pay when the employee is unable to work due to <br />illness, the necessity for medical, chiropractic or dental care, childbirth or pregnancy, sickness of <br />a member of the employee's "immediate family" subject to the City's Personnel Policies. <br />10.4 In the event of serious illness in the immediate family of the employee, sick leave may be used to <br />provide necessary care which requires the presence of the employee, and which cannot be <br />obtained in another manner. For purposes of this Subsection, it shall be subject to the City's <br />Personnel Policies. <br />10.5 The Employer may require the employee to submit a medical certificate, in a form satisfactory to <br />the Employer, in the event of absence of three (3) days or more, or If the Employer reasonably <br />suspects misuse or excessive use of sick leave. Abuse of sick leave may be cause for disciplinary <br />action. <br />10.6 Employees shall notify their supervisor by telephone or other means at the earliest opportunity <br />when they will be unable to report to work as scheduled because of illness. Such notice shall be <br />given as soon as possible prior to the start of the shift which will be missed because of illness or <br />injury. <br />10.7 In the event of an employee death or disability, severance pay otherwise payable under this <br />Article shall be paid to the employee's beneficiary, as in connection with any insurance policy <br />issued under the provisions of Article 15, such pay shall be computed at the employee's regular <br />straight time hourly rate of pay at the time of the disability, or death. <br />10.8 For employees receiving weekly benefits under the Workers Compensation Act, accumulated, <br />unused sick leave may be used to make up the difference between such benefits and the <br />employee's normal net earnings each period. <br />10 <br />
