and the City Clerk the reason for his or her condition and if the absence is of <br /> more than three (3) days duration, the City may require the employee to <br /> submit a medical certification for the absence. The medical certification <br /> shall contain the medical provider's diagnosis, when the employee is likely <br /> to return to work and any limitation on the employees work upon return to <br /> service. The City Council may waive these requirements. <br /> 4. Penalties: Using or claiming sick leave for a purpose not authorized <br /> by these policies may be cause for disciplinary action, up to and including <br /> discharge, as outlined in the Discipline section of these policies. <br /> 5. Probationary Period: During the first six (6) months of the <br /> probationary period following an original appointment, an employee may <br /> earn sick leave but shall not be entitled to use sick leave. Upon successful <br /> completion of the six (6) month probationary period the employee is entitled <br /> to sick leave accrued from the start of probationary employment. <br /> 6. Sick Leave Benefit Payments Upon Separation: Upon separation from <br /> employment, either due to retirement, voluntary or involuntary termination <br /> for whatever reason, full-time and regular part-time employees with five to <br /> nine (5-90) years of service with the City of Pequot Lakes shall receive <br /> monetary compensation in the amount of ten percent (10%) in cash of <br /> unused sick leave up to 960 hours. Employees with ten (10) or more years of <br /> service with the City of Pequot Lakes shall receive monetary compensation <br /> in the amount of twenty five percent (25%) in cash of unused sick leave up <br /> to 960 hours. The City Clerk shall maintain a record of all sick leave earned <br /> and used. <br /> Accrual During Leave <br /> For the purpose of accumulating additional vacation or sick leave, an <br /> employee using earned vacation leave or sick leave is considered to be <br /> working. <br /> Military Leave <br /> 16 <br />
