In order to be eligible for reimbursement, a uniform expense reimbursement <br /> request must be received prior to December 1 of the calendar year. <br /> 13.3 The following items shall be furnished by the City of Pequot Lakes: <br /> 1) Badges, Collar Brass, and Name Tags <br /> 2) Patches, Insignias, Flags <br /> 3) Ammunition for duty and qualifying <br /> 4) Non-lethal weapons such as taser guns, handcuffs, pepper spray, mace, etc. <br /> 5) Safety Vest (bullet proof) <br /> 6) Duty weapon/holster <br /> 7) Initial sets (2) of uniforms for new employees to include shirts (summer, <br /> winter), pants,jacket/coat. <br /> 13.4 In the event there are changes to a specific style or color of uniforms and/or <br /> equipment, and it is mandated by the City of Pequot Lakes,the City shall purchase <br /> all necessary uniforms (minimum of two sets) and/or equipment. Should a <br /> uniform become damaged while in the duty to the City it shall be replaced at no <br /> cost to the employee or a decrease in the employee's annual uniform allowance. <br /> `.. ARTICLE 14 <br /> INSURANCE <br /> 14.1 Health and Hospitalization Insurance. The Employer shall provide a group <br /> medical/surgical plan for employees and their dependents. The Employer shall <br /> contribute eighty-five percent (85%) of the monthly premium for the coverage <br /> elected by the Employee, either single or dependent coverage, and the employee <br /> shall pay fifteen percent(15%) of the monthly premium. <br /> 14.2 Dental Insurance. If the City determines to provide a policy of group dental <br /> insurance coverage for employees, employees may participate upon payment of <br /> the monthly premium by the employee. <br /> 14.2 Employees who work an average of thirty (30) hours per week or more shall <br /> receive pro-rata Employer contribution based on their anticipated scheduled hours <br /> of work for the year. <br /> 14.3 Retirees. The Employer will pay on behalf of an employee who retires from the <br /> City at a time when the employee is eligible for and will immediately begin <br /> receiving a PERA retirement annuity, a portion of the premium for continued <br /> coverage under the Employer's health and dental insurance programs, for up to <br /> three (3) consecutive years beginning with the employee's retirement from the <br /> 22794-1 (9.16.08) 14 <br />
