STATEMENT OF WORK <br /> THIS STATEMENT OF WORK("SOW")describes the Services,and the related deliverables and milestones to be performed by <br /> Benovate("Benovate"),for City of Pequot Lakes("Client"),subject to the terms and conditions of the Health Services Agreement <br /> (the"HSA")by and between Renovate and NJPA dated on December 1,2015.All capitalized terms not defined herein shall have <br /> the meanings assigned in the HSA.In the event of a conflict between the terms of this SOW and the HSA,this SOW shall prevail. <br /> The agreed upon effective date is January 3,2017.It is understood that certain obligations of Benovate below are dependent on <br /> Client providing certain data,information,or assistance to Benovate from time to time,and that such cooperation will be essential <br /> to Benovate's meeting its obligations. <br /> Definition: For the purposes of this Statement of Work,Eligibility is defined as: <br /> • All active employees on the employer-sponsored health plan—Eligible for participation&incentives <br /> Benovate will provide the following: <br /> 1. Program Communications and Marketing <br /> In order to successfully engage Client's employees,Benovate will coordinate and execute marketing strategies targeted at the <br /> employee level.These campaigns are built to educate and engage prior to the official launch date of the employee's access to the <br /> platform. <br /> The following items represent the first wave of education/engagement communications.Additional education and marketing <br /> campaigns will be executed throughout the term of the agreement,on an as-needed basis.All communications will be sent digitally, <br /> unless otherwise noted.The Client may request print material for an additional fee,to be determined on a per project basis. <br /> • Program Onboarding <br /> o "Teaser"launch campaign <br /> o Platform launch emails,minimum 2 <br /> o Letter of Support/program endorsement template <br /> o Program Overview Guide and supporting documents(FAQ etc.);Annually <br /> o Employee newsletter/internal website write up provided to Client for indusion; Minimum quarterly <br /> o Poster PDFs; Minimum 2 per campaign <br /> o Benovate Beat email;monthly <br /> o Health Champion campaigns <br /> Deliverables:Initial outreach communications sent prior to platform launch and ongoing communications sent thereafter. <br /> Projected Delivery Date(s):Marketing Campaign to begin no later than 21 days prior to the January 3,2017 start date and is <br /> contingent upon receiving an up-to-date employee census and incentive limits. <br /> 2. Renovate Health Engagement Platform <br /> The Engagement Platform is a HIPAA-compliant,personalized portal accessible from current smartphones and tablets running <br /> current software,as well as modem PC's running current software(Software and Hardware Requirements list available upon <br /> request).The platform will provide the following: <br /> • Health awareness <br /> • Symptom education <br /> • Stress and Lifestyle surveys <br /> • Daily activity/points tracking <br /> • Reporting: Engagement,Incentives,and Health/Lifestyle Risks(Expanded functionality coming in future versions) <br /> o All reporting provided to Client shall be aggregate data only,and will not include individual data Only <br /> partiapants will have access to their own individual data though Benovate's platform. <br /> • Health Risk Assessment through US Health Center PHD <br /> • Data Synthesis Analysis(Predictive Modeling) <br />
