The <br />premium breakdown is as follows. <br />Three for Free Plan <br />Family Rate =$868.22 per employee per month <br />Employer's Share is 85% _$738.00 per month <br />Employee's Share is 15% _$130.22 per month <br />Single Rate =$609.66 per employee per month <br />Employer's Share is 85% _$518.22 per month <br />Employee's Share is 15% _$91.44 per month <br />HSA Plan <br />Family Rate =$821.16 per employee per month <br />Employer's Share is 85% _$698.00 per month <br />Employee's Share is 15% _$123.16 per month <br />Employer's HSA Contribution =$200 per month <br />Single Rate =$573.04 per employee per month <br />Employer's Share is 85% _$487.08 per month <br />Employee's Share is 15% _$85.96 per month <br />Employer's HSA Contribution =$150 per month <br />If you have any questions, let me know. <br />Thank you. <br />
