PREVENTATIVE MAINTENANCE CONTRACT <br /> ADDENDUM <br /> Arrow Lift Accessibility (ALA)will furnish a PREVENTATIVE MAINTENANCE <br /> INSPECTION(S) on the following equipment located at City of Pequot Lakes: <br /> 4638 County Road 11, Pequot Lakes, MN. <br /> Manufacturer: Vertechs LULA <br /> Service Contract Options: Select One <br /> One (1) year contract, One Service Inspection - $ 135.00/yr <br /> Five (5) year contract, One Service Inspection - $ 125.00/yr <br /> Annual payment(s), payable in advance, the Contract price may be adjusted on <br /> the anniversary date of this agreement. <br /> Contract is valid from the date of a signed contract and thereafter until this <br /> agreement is terminated by ninety (90) days written notice to that effect, given by <br /> either of the parties hereto to the other party. <br /> IT IS EXPRESSLY AGREED that payment of all sums then due hereunder is a <br /> Condition precedent to the future rendering of services by Arrow Lift Accessibility <br /> and ALA reserves the right, at our option, to suspend services until all payments <br /> then due are made, at any time you are in default in making payments as <br /> stipulated. Suspension of services by ALA shall not be deemed to be a breach of <br /> this agreement and we shall not be liable for any loss or cost of damage resulting <br /> from suspension. Not withstanding any provision of this agreement to the <br /> contrary, if you fail to cure such default within 30 days following receipt of a <br /> suspension notice, we may terminate this agreement upon written notice, <br /> effective immediately, without prejudice to any other rights or remedies which we <br /> may have at law or in equity. <br /> This service inspection(s) to be performed between the hours of 8 a.m. and 5 <br /> p.m. Monday to Friday (excluding holidays). As conditions warrant, in our sole <br /> judgement, we may carry out the following services at the time of inspections: <br /> where applicable: <br /> • Cleaning or oiling of machine, motor, signal devices, interlocks and <br /> controllers. <br /> • Greasing or oiling guides. <br /> • Necessary cleaning and lubrication of rails. <br /> \.• • Minor adjustments as deemed necessary by us at the time of regular <br /> inspection. <br />
