S.P. 1810-92(T.N. 371) <br /> Agreement Number 1000912 <br /> A. Form: The Work Orders will be in substantially the same form as Exhibits C(1) <br /> and C(2). <br /> B. Contents/Amendment- The Work Order, and any agreements or documents <br /> executed or prepared pursuant to it: <br /> 1. Will include: <br /> a. The design plans; <br /> b. Any available, applicable design details; <br /> C. Any Right of Way/easement documents at the UDS location; <br /> d. The scope of the Utility Work; <br /> e. The party responsible for performing the Utility Work; <br /> f. The lump sum or estimated amount of the Utility Work, as applicable; <br /> g. The Utility Work schedule; <br /> h. The Utility Owner's special scheduling considerations (e.g., winter <br /> gas loads and summer electric loads): <br /> i. The Utility Owner's Inspection fees; and <br /> j. Any other conditions applicable to the Work Order that this MUA <br /> does not already address. <br /> 2. May also amend the terms and conditions of this MUA upon inclusion of <br /> appropriate language describing the change. <br /> C. Negotiation: MnDOT, the Utility Owner, and the Contractor must promptly meet <br /> and confer to resolve any comments or disagreements regarding the Work Order <br /> through good faith negotiation. The Contractor will prepare the Work Order after <br /> the parties resolve the comments or disagreements. <br /> D. Submission/Response/Execution: The Contractor will provide notice of the Utility <br /> Work and its schedule in a Work Order to the Utility Owner. The Utility Owner will <br /> respond (by executing or commenting on problems) to all Work Orders no more <br /> than 14 Calendar Days after receiving them. The Contractor shall submit four <br /> originals of each Work Order (including any exhibits) to Mn/DOT for Approval upon <br /> execution by the Utility and Contractor. Mn/DOT will respond with comments no <br /> later than 10 Working Days after receipt. <br /> E. Standards: All standards, procedures, terms, and conditions set forth in this MUA, <br /> and in all MnDOT Utility Permits issued subsequent to the execution of the Work <br /> Page 8 of 28 <br />
