FEB-29-2012 12:01P FROM:HART ANIMAL SHELTER 2188296866 T0:12185685860 P.2 <br />� • The municipality will be responsible for any veterinary cost incurred during <br />impound period. <br />• Impound fees will be collected from the owner at HART. Claimed animals will be <br />released by HART only upon payment from the owne�. <br />• The municipality will pay HART accordi�g to the fee schedule set forth between <br />HART and ihe municipality to include: <br />a. Impound fees: $14.00 per day for each animal impounded; maximum of <br />seven (7) days <br />b. A$25.00 administrative fee will be assessed to each anlmal impounded <br />c. Veterinary charges as charged by the clinic <br />d. Euthanasia fees for unadoptable animal <br />e. Quarantined animals (e.g.) ag�ressive, rabies observation, will be charged <br />for special care handling at the rate of $50.00 pe� day impound plus a <br />onetime $50.00 administrative fee, for a maximum of ten (10) days. <br />3. The undersigned have read and understand this contractual service agreement. They <br />agree to the terms and condltlons stated. <br />� Municipallty Representative <br />HART Representative <br />Summary of General Provislons: <br />Title <br />Title <br />• HART will provide the municipality with proof of insurance. <br />• Animal care will be in compliance to standards listed in the Minnesota Animal Welfare <br />Statues. <br />• Sick or injured anirnals will be evaluated fo� medical needs and t�eated if required. <br />HART will not be responsible for animal that arrive sick, injured or die as a result of <br />arrival condition, while in impound. <br />• HART will invoice the munlcipality monthly. <br />� <br />
