Page 5 of 8 <br /> B. What to do at the Scene of an Accident <br /> 1. Report all vehicle accidents. <br /> 2. Complete a Driver's Accident Report <br /> C. City of Pequot Lakes will investigate all accidents involving City of Pequot Lakes <br /> vehicles. The following shall be considered: <br /> 1. It is unlawful to leave the scene of any accident if you are involved in the <br /> accident, without furnishing your name, address and vehicle information to the <br /> other driver. Any hit and run accident, on private or public property, should be <br /> reported to the local police, sheriff or state patrol office with jurisdiction. <br /> 2. Accidents involving a pedestrian and a City of Pequot Lakes vehicle, or an accident <br /> involving a City of Pequot Lakes employee who is struck by a vehicle, should also <br /> be reported to the local police, sheriff or state patrol office within the jurisdiction. <br /> D. The Safety Committee will act as the Accident Review Committee. <br /> 1. All accidents will be reviewed by the Safety Committee to determine if the accident <br /> was preventable or non-preventable. <br /> 2. The Accident Review Committee will use the City' guidelines for determining <br /> whether an accident is preventable or non-preventable. <br /> 3. All accidents that are voted as preventable will be counted as an accident on the <br /> employee's record. <br /> 4. The driver has the right to appeal, in writing, if there is disagreement with the <br /> Committee's action. Appeals shall be made to the Council liaison. <br /> Section 3 4: Definition of an Acceptable Driver <br /> Any employees driving on behalf of the City needs to maintain a valid state driver's license, and a <br /> rating of"Poor" or higher in accordance with the "Acceptable Driver"definitions listed in the <br /> policy. <br /> A 36-month period is reviewed to determine an employee's driver rating.
