The premium breakdown is <br /> as follows. <br /> Three for Free Plan <br /> Family Rate= $876.58 per employee per month <br /> Employer's Share is 85% = $745.10 per month <br /> Employee's Share is 15% = $131.48 per month <br /> Single Rate=$612.82 per employee per month <br /> Employer's Share is 85%= $520.90 per month <br /> Employee's Share is 15%= $91.92 per month <br /> HSA Plan <br /> Family Rate = $829.52 per employee per month <br /> Employer's Share is 85% = $705.10 per month <br /> Employee's Share is 15%= $124.42 per month <br /> Employer's HSA Contribution= $200 per month <br /> Single Rate= $576.20 per employee per month <br /> Employer's Share is 85% = $489.78 per month <br /> Employee's Share is 15% = $86.42 per month <br /> Employer's HSA Contribution= $150 per month <br /> For 2015, the health insurance rates for the Teamsters Local 346 Health Fund will increase by <br /> 10.00% from 2014.
