After participating in Option 1 or Option 2, employees may earn personal time off(PTO) <br /> hours by participating in a variety of wellness activities suitable for all levels and earning <br /> points as outlined on the City of Pequot Lakes 2015 Wellness Activity Descriptions and <br /> Tracker. If an employee is participating in more than one activity simultaneously, <br /> exercise for one activity cannot be counted for another activity. No double dipping! <br /> Incentives <br /> Employees who choose to participate in Option 1 and submit the Proof of Completion <br /> Form and documentation will receive a $10 gift card to a business within the City of <br /> Pequot Lakes. Employees who choose to participate in Option 2 and submit the Proof of <br /> Completion Form and documentation will receive $100. <br /> Full-time employees who choose to participate in the Wellness Activity Tracker, will <br /> earn 2 hours of personal time off for every 50 points tracked. Part-time employees will <br /> earn 1 hour of personal time off for every 25 points tracked. The personal time off <br /> maximums for the year are 8 hours for full-time employees (200 points) and 4 hours for <br /> part-time employees (100 points). Time off will be credited in 50 and 25 point <br /> increments respectively. <br /> To receive incentives, employee must be actively employed by the City of Pequot Lakes <br /> at the time incentives are distributed. <br /> Please note the IRS considers all cash or cash equivalent incentives to be taxable <br /> compensation. If an employee receives a gift card or money for participating in the <br /> City's Wellness Program, the incentive will be considered taxable income and included <br /> on the employee's W-2 form. <br /> Proof of Completion <br /> The attached Wellness Program Proof of Completion Form and Wellness Activity <br /> Tracker must be completed and submitted along with documentation to Nancy Malecha. <br /> Completed forms can be submitted at any time during the year but no later than <br /> December 18, 2015. If the fetm-and documentation is complete, the incentive award(s) <br /> will be processed. <br /> Disclosure <br /> The Pequot Lakes City Council reserves the right to change the Wellness Program and <br /> incentives from year to year based on participation and available funding. <br />
