Health Insurance Council Memo <br />October 5, 2010 <br />Page 2 of 3 <br />1. $1500/$3000 Deductible Three for Free Plan <br />• The City pays 85% of the plan premium and the <br />employee pays 15% of the plan premium <br />2. HSA $2500/$5000 Deductible Health Plan <br />• The City pays 85% of the plan premium and the <br />employee pays 15% of the plan premium <br />• In January 2011, the City will make a HSA <br />contribution of $2,000 for each single contract <br />and $3,400 for each family contract (This one <br />time up -front payment is an incentive for <br />employees to consider an HSA health plan). <br />• In 2012 and beyond, the City will make monthly <br />HSA contributions of $150 per month for each <br />single contract and $200 per month for each <br />family contract <br />The Committee is asking the Council to consider offering <br />the two health plan options to all City employees. If the <br />Council moves forward with these two plans, open <br />enrollment will begin on November 1, and run through <br />November 30th <br />There has been one city employee meeting informing <br />employees of the two health plan options with the <br />understanding that this is a recommendation to the Council <br />from the Health Insurance Committee.
