Citv of Peauot Lakes Citv of Peauot Lakes Personnel Policies <br /> Title: Leaves Effective Date: [date adopted] <br /> Effective Date(s)of Amendment(s): <br /> If the unpaid leave of absence is not approved or the employee fails to request additional leave, <br /> the employee will be considered to have voluntarily resigned. <br /> If circumstances beyond the employee's control prevented the employee from requesting <br /> additional leave,a retroactive leave request may be allowed,subject to the City Council's approval. <br /> 10.6 Job Related Injury or Illness <br /> All employees are required to report any job-related illnesses or injuries to their supervisor <br /> immediately(no matter how minor). If a supervisor is not available and the nature of injury or <br /> illness requires immediate treatment,the employee is to go to the nearest available medical <br /> facility for treatment and, as soon as possible, notify his/her supervisor of the action taken. <br /> In the case of a serious emergency,911 should be called. If the injury is not of an emergency <br /> nature, but requires medical attention,the employee will report it to the supervisor and <br /> make arrangements for a medical appointment. <br /> Worker's compensation benefits and procedures for returning to work will be applied according <br /> to applicable state and federal laws. <br /> 10.7 Marrow Donation <br /> Employees working an average of 20 or more hours per week may take paid leave,not to <br /> exceed 40 hours unless agreed to by the City,to undergo medical procedures to donate bone <br /> marrow. ' <br /> The City may require a physician's verification of the purpose and length of the leave requested <br /> to donate bone marrow. If there is a medical determination that the employee does not qualify <br /> as a bone marrow donor,the paid leave of absence granted to the employee prior to that <br /> medical determination is not forfeited. <br /> 10.8 Reasonable Unpaid Work Time for Nursing Mothers � <br /> Nursing mothers will be provided reasonable unpaid break time to express milk for nursing her � <br /> child for one year after the child's birth.The City will provide a room (other than a bathroom) ' <br /> as close as possible to the employee's work area,that is shielded from view and free from <br /> intrusion from coworkers and the public and includes access to an electrical outlet,where the � <br /> nursing mother can express milk in private. '� <br /> 10.9 Pregnancy and Parenting Leave ! <br /> Employees who work twenty(20) hours or more per week and have been employed more than '' <br /> one year are entitled to take an unpaid leave of absence under the Pregnancy and Parenting I'� <br /> Leave Act of Minnesota.
