PEQUOT LAKES COMMU1vITY LIBRARY ADMINISTRATIVE POLICY <br /> PATRON RESPONSIBILITY AND ELIGIBILITY POLICY <br /> Responsibility: Materials Coordinator <br /> Revised: June 20,2017 <br /> Any resident of the Pequot Lakes area, even though their main residence is elsewhere, may be <br /> issued a Pequot Lakes Community Library card after paying the fee. T'he one-time fee for this card <br /> is $5.00. Residents should present a utility bill, driver's license or other such information as proof <br /> of eligibility. <br /> Pequot Lakes Community Library will issue a library card with regular bonowing privileges to <br /> homeless people or people in shelters. Identification requirements are waived for purposes of <br /> issuing a card. The address of the Library is used as the address of the patron. �� <br /> Minor children may have a library cazd with the approval of their parents.Recognizing that a wide <br /> variety of family structures exist, there are some circumstances where minor children (under age <br /> 18)with difFerent resident addresses may have two barcoded library cards and two patron records. � <br /> This includes: Minor children who are members of separated families and who reside at two <br /> households. Quite often in these instances, the cards are kept at the separate households and ' <br /> different parents take responsibility for control of each card. Also,minor children in foster homes <br /> would fall into this category. A child may have one card for the home address and another for the <br /> foster family address. <br /> Patrons presenting a valid library card are eligible for borrower's privileges of the Pequot Lakes <br /> Community Library: <br /> • Patrons are responsible for all materials charged out on their cards and for any charges <br /> incurred. <br /> • Patrons must notify the Library of lost or stolen cards and of name or address changes. <br /> After checking to be sure the patron has no outstanding materials listed on the old card, a <br /> replacement card can be issued for a chazge of$1.00. <br /> • Responsibility for control of a child's library card and selection of library materials lies <br /> with the parent or legal guardian. <br /> Borrowing and registration privileges may be suspended if the patron fails to return materials or <br /> moves out of the Pequot Lakes azea. <br /> • IF THE PATRON HAS 5 OVERDUE MATERIALS, the volunteer should explain that <br /> Library policy prevents renewal or check out of additional materials until the overdue <br /> materials are returned. <br /> 21 <br />
