1.1.1. Visit FACILITIES daily and charge City for the daily <br />visits based on the Monthly rate. <br />1.1.2. Prepare and submit on behalf of CITY all required <br />MPCA and MDH monthly operations reports as required <br />by both regulatory agencies. <br />1.1.3. Perform chlorine, fluoride, and phosphate testing on a <br />weekly basis or as otherwise required by the MDH. <br />1.1.4. Collect and submit fluoride and bacteriological samples <br />to MDH on a quarterly basis. <br />1.1.5. Collect and submit groundwater monitoring well <br />samples three (3) times a year as required by MPCA <br />1.1.6. Prepare and submit monthly regulatory reports. <br />1.1.7. Perform semi - annual fire hydrant flushing, lubrication, <br />flag installation /removal. <br />1.1.8. Prepare annual Consumer Confidence Report, and <br />distribute and submit Consumer Confidence Report as <br />required by MDH. <br />1.1.9. Turn on /off curb stops valves as requested. <br />1.1.10. Respond to water /sewer complaints, notify CITY of <br />water /sewer complaints, and arrange for repairs to <br />address the water /sewer complaints by a qualified <br />vendor of CITY's choice, at CITY's direct expense and <br />at CITY's direction. <br />1.1.11. Respond to water /wastewater related alarms, notify <br />CITY of water /sewer alarms, and arrange for repairs to <br />address water /sewer alarms and equipment necessary <br />to complete repairs by a qualified vendor of CITY's <br />choice, at CITY's direct expense and at CITY's <br />direction. <br />1.1.12. Perform day -to -day equipment maintenance. <br />1.1.13. Maintain water /wastewater related record keeping. <br />1.1.14. Record hour meter readings at wastewater lift <br />stations, Main lift daily, all othersl time per week. <br />1.1.15. Record water usage readings, water tower level, and <br />chemical usage daily. <br />
